- Pag-ibig Virtual Account
- HR/Employer valid contact details for credit investigation (contact no. and email address)
- 1×1 Picture 6 pieces (white bg, front facing, no accessories)
- Marriage Contract if Married (NSO, PSA, or local civil registrar photocopy is accepted)
- Birth Certificate (NSO, PSA, or local civil registrar photocopy is accepted)
- Photocopy of Two Valid IDs with 3 specimen signatures (SSS ID, POSTAL ID, PRC ID, VOTERS ID, DRIVER’S LICENSE, PASSPORT)
- Photocopy of Spouse ID (if Married) with 3 specimen signature
- Verified BIR 1904 Form / TIN Verification slip (with stamp verified by BIR)
- Verified BIR 1905 Form (registration of information update/correction)
- Signed MSVS (Membership Status Verification Slip)
- Conformity for non-relative (if the Co-Borrower is not on the second civil degree of consanguinity or affinity)
- Pag-Ibig Borrower’s Validation
- Pag-Ibig Health Questionnaire (if Loanable Amount is 2M)
- Pag-Ibig Health Questionnaire with Medical Certificate (for Borrowers up to 60 years old)
- Pag-Ibig MID number
- Housing Loan Documents (to sign)
LOCALLY EMPLOYED CLIENTS
- Latest original Certificate of Employment and Compensation with company letter head
- Monthly pay slip (whole month, from the time of reservation up to time of submission of docs in Pag-Ibig)
- Latest ITR/Cert. of Tax Witheld – BIR Form 2316 (If no tax deduction on Payslip)
OFW CLIENTS
- Latest Contract of Employment with POEA seal / Salary Certificate
- Latest Remittance Form(with sender & receiver’s name and origin of remittance) or Bank Statement or Allotment Slip with Bank Statement (if Seaman)
- ID of Employer with contact no. & signature (If the employer is not a firm)
- Photo copy of Passport with 3 specimen signature
- Photo copy of Seaman’s Book with 3 specimen signature (if applicable)
- Photo copy of two valid ids with 3 specimen signature (SSS ID,POSTAL ID,PRC ID,VOTER’S ID,DRIVER’S LICENSE,PASSPORT)
- Photocopy of Spouse Valid ID with 3 specimen signature
- Special Power of Attorney from the Phillipine Consul or Locally notarized (If Locally notarized, must provide Departure & Arrival stamp)
- Valid ID of Attorney-in-fact with 3 specimen signature
SELF-EMPLOYED CLIENTS
- BIR Certificate of Registration Form 2303
- Latest Mayor’s Permit and DTI Permit
- Latest Annual Income Tax Return with Audited Financial Statement
- Tax Receipt
- Picture of the business/store
- Sketch of business location
