  9. Verified BIR 1905 Form (registration of information update/correction)

  11. Conformity for non-relative (if the Co-Borrower is not on the second civil degree of consanguinity
    or affinity)
  12. Pag-Ibig Borrower’s Validation
  13. Pag-Ibig Health Questionnaire (if Loanable Amount is 2M)

  2. Monthly pay slip (whole month, from the time of reservation up to time of submission of docs in Pag-Ibig)
  3. Latest ITR/Cert. of Tax Witheld – BIR Form 2316 (If no tax deduction on Payslip)
  1. Latest Contract of Employment with POEA seal / Salary Certificate
  2. Latest Remittance Form(with sender & receiver’s name and origin of remittance) or Bank Statement or Allotment Slip with Bank Statement (if Seaman)
  3. ID of Employer with contact no. & signature (If the employer is not a firm)
  4. Photo copy of Passport with 3 specimen signature
  5. Photo copy of Seaman’s Book with 3 specimen signature (if applicable)

  3. Latest Annual Income Tax Return with Audited Financial Statement

GENTLE REMINDER TO OUR CUSTOMERS

  1. All payments made by clients are non-refundable and non-transferrable
  2. If the client misses one month of equity payment, they will be put through a forfeiture procedure
  3. All clients who will reserve will be subject to penalty if their equity deadlines are not met
  4. The client must submit all the required documents within the first 2 months to avoid forfeiture
  5. The client must understand that some documents may be asked again due to expiry (ex. Certificate of Employment, Payslip, etc.)
  6. The client must understand that although we are aware that some documents will expire, they must still comply and submit all of their Pag-IBIG requirements within the first 2 months for PRE –SCREENING AND ENCODING PURPOSES
